Vision Insurance option
The Eye Team Optical Shoppes and Drs. Schild and Collins office participate with many vision insurance plans. For information about whether we participate with your plan, you may consult your benefit book, your human resources representative, or you may call our office.
Discounts: Special discounts are offered
for participating employers. Please consult your human resources
representative as to whether your employer is a participating company.
** All discounts are applied to your out of pocket balance after
insurance.
Submitting Insurance:
Our offices are able to submit many vision and medical claims for our patients. Simply present a recent, up to date, insurance card with the following information:

| ID number |
| Group number |
| Mailing address for claims |
We will also need the covered party and the patient’s up to date information including: Name, address, phone, birthdate, social security number, and employer. For the best success in having a claim paid in a timely manner, both the patient and our office should be well informed of the available benefits and criteria that must be met for the claim to be processed. Although our office has access to electronic filing and most forms needed for submitting claims, the patient should be aware of any paperwork that must be retained from their human resource office to successfully file a claim.
